The origin

He built an exit ramp — and never used it

A pharmacist’s personal crisis became a billion-dollar industry. Just not in the way he intended.

Hon Lik was a Chinese pharmacist watching his father die of lung cancer while going through three packs a day himself. So he turned his apartment into a lab. His idea: separate the nicotine from the smoke. Give heavy smokers the hit without the tar.

When he filed the patent in 2003, the goal was harm reduction — a medical off-ramp for people already deep in cigarette addiction. Not a new habit for people who’d never smoked.

Lik remains a dual user today — still smoking, still vaping. The man who invented the device meant to free him couldn’t free himself. Meanwhile, a generation that had never touched a cigarette learned to inhale anyway.

What happens on a single hit

Vaping bypasses most of the warning signals that harsh smoke triggers. Here’s the sequence.

The lung is built to pass gases directly into blood — which is exactly why vaping works so efficiently, and why the damage is so easy to miss.

The device

Not water vapour — aerosol

The coil heats propylene glycol and vegetable glycerin into a fine aerosol. That process can produce reactive compounds — including formaldehyde and acrolein — before the mist even reaches your airways.

The lungs

Cilia slow down

The aerosol coats your airway lining and can slow cilia — the tiny hair-like structures that clear debris from your lungs. Inflammation builds in the alveoli over months, quietly reducing the tissue flexibility that lets you breathe deeply.

What the research actually says

Not a lecture. Just what works, what doesn’t, and why most quit attempts fail in the first week.

Here’s the thing most quit plans get wrong: they treat vaping like cigarettes. Same advice, same patches, same logic. But the nicotine delivery is faster, the ritual is more embedded in daily life, and the perceived harm is lower — so the psychological hooks are different.

The research on what actually works is more nuanced than “just stop.”

Evidence: Strong

Combination NRT + behavioural support

Using nicotine replacement (patch plus a fast-acting form like gum or lozenge) alongside structured support — even just a text-based programme — roughly doubles quit rates versus going cold turkey. The patch handles baseline craving; the gum handles spikes. Neither alone is as effective as both together.

Evidence: Strong

Varenicline (Champix / Chantix)

The prescription medication that blocks nicotine receptors while reducing withdrawal. Trials consistently show it outperforms NRT and bupropion. It requires a GP conversation, but for heavy vapers who’ve tried and failed other methods, it’s worth raising.

Evidence: Mixed

Cold turkey

Most people try this first. And some succeed — particularly those with strong social support and a concrete reason to quit. But relapse within the first week is common, and repeated failed attempts can erode confidence in ways that make the next attempt harder.

Evidence: Weak

Vaping to quit smoking (and then stopping)

The theory is sound — use a less harmful delivery method to step down. In practice, most people who switch don’t quit vaping either. It works best as a bridge, not a destination, and works better when paired with a clear reduction schedule.

The single biggest predictor of a successful quit isn’t the method — it’s whether you’ve told someone. Social accountability, even informal, significantly improves outcomes. If you’re quitting, say it out loud to one person.

The takeaway

The device was never the problem

Hon Lik’s invention wasn’t wrong. What happened to it was.

Vaping is not cigarettes. That much is probably true. But “less harmful than a Marlboro Red” turned out to be a remarkably low bar to clear — and an easy one for an industry worth $45 billion to hide behind.

The people most affected aren’t the heavy smokers Hon Lik was trying to help. They’re a generation who never had a reason to start, handed a sleek device with enough nicotine to rewire their dopamine response before they’d finished their first year of uni.

“The question was never whether vaping is safe. It’s whether a generation that never smoked needed to find out.”

If you vape and want to stop, the tools exist. If you don’t vape, the science isn’t complicated: the cleanest aerosol is still not clean air. And if you know someone deep in it — the most useful thing you can do is not make them feel stupid for starting.
